Russel치주지수에 따른 치조백선의 변화에 관한 방사선학적 연구

Abstract

저자는 치주질환의 진행에 따라서 초기와 중증병소, 치조골의 흡수방향과 흡수량 및 치조
조직 파괴능 등을 평가하는데 기준이 될 수 있는,치조정부위의 치조백선의 위치와 치조와의
부위별 치조백선의 두께를 관찰하고자 본 연구를 실시하였다. 치조백선에 영향을 미칠 수
있는 전신적 요인이 없다고 판단되는 17세에서 39세 사이의 본원 내원환자를 연구대상으로
하여, 개변한 Russel의 치주지수를 이용하여, 상악과 하악의 전치부와 대구치부에 대해 평행
법을 이용한 치근단X선사진을 얻었다. 이 X선사진들을 환등기를 이용하여 20배로 즉대, 투
사하여 각 치아별로 치경부, 치근중앙부, 치근단부에서 치조백선의 두께를 계측하였고, 백악
법랑경계부로부터 치조정 치조백선의 길이를관찰하여 다음과 같은 결과를 얻었다.
1. Russel의 치주지수 0군, 1군 및 2군에서 치조백선의 두께는 치경부, 치근단부, 치근중앙
부의 순으로 두꺼웠으며, 전치부보다는 구치부에서 더 두꺼웠다.
2. 치주지수 0과 1군에서 치조백선의 두께는 대체로 근사하였으나, 치주지수 2군에서는 치
주지수 0군과 1군에 비해 치경부에서만 감소하였으며, 치주지수 6군에서는 치경부에서만 더
욱 감소하였다..
3. 백악법랑경계부와 치조정 치조백선의 길이는 치주지수 0군과 1군에서는 1.5㎜이내였으
나, 치주지수 2군에서는 1.5㎜이상으로 증가하였으며, 치주지수 6군에서는 더욱 증가하였다.

The aim of this study was to investigate the thickness of alveolar lamina dura and
alveolar bone height by the change of periodontal status. The periapical radiograms of
the 853 cases were taken by paralleling technique.
The conclusions from this study were as follows:
1. In the normal periodontal status, the alveolar lamina dura was the thickest at
alveolar crest, followed by apex, midroot, and the average thickness of the posterior
teeth was thicker than that of the anterior teeth.
2. In Russel's Periodontal Index 0 and 1, no significant differences of thickness were
found to be 3 areas. In Russel's Periodontal Index 2 and 6, the thickness of alveolar
lamina durra at alveolar crest was decreased, only.
3. In Russel's Periodontal Index 0 and 1, distance from the cemento-enamel junction
to the alveolar crest was within 1.5mm. In Russel's Periodontal Index 2 and 6, the
distance was increased over 1.5 mm.
